Why do they tell you not to push to hard when your taking a dump?

You can actually have something called vagual vaggeling which sounds crazy but it pinches the vagus nerve running to the heart and can cause an extreme drop in blood pressure and pulse causing you to pass out and possibly die. Seriously...it's crazy..

The former is rare unless you have a heart condition and I've never heard of the latter happening...but who would.
