At least with drowning, you will panic, but then all of a sudden- thats it- your brain gets no oxygen and then lights out...
Diseases are terrible too- but there are drugs to control pain.
But being buried alive...oh god, the very thought freaks me out...

Bone cancer or pancreatitis. Both very painful and very long. starvation or dehadration... hurts so bad you wanna kill your self.
