Insurance fraud?

i had a surgery. The people in attendance told me that my insurance covers the surgery but, because i have a document missing, i could have my money subsidise only after the claim is sent.So i paid full price $350 instead of $96.
Now i receive a communication from my insurance telling me that my claim has be sent . But what they give me back is $1040.
What am i supposed to verbs?
Did the office ask for more that they spent?
Is it a mistake from the insurance?
What should i do?
Thanks for anything that could make this clearer for me.

Answers:    hello, very well i think that the doctors office solitary billed u for him doing the surgery & the other things like the anesthesia, the surgery room, if u had to stay contained by the hospital then the hospital room price for each time u stayed wasn't included in the whole bill so u can phone call the insurance company up & ask them to send u an itemized bill so u can have it for your accounts & that it wasn't sent to u with the check. then linger & see what's on the bill before u do anything & i'm sure that u were compensated for adjectives that was done & that the doctor's bill was the one and only thing that u know of that got remunerated so good luck!
