I don't know ample about you to suggest the best form of counselling but I can recount you that cognitive behaviour psychotherapy (cbt) is very influential at dealing with addiction.
you need to identify your strengths and your weakness and focus on the strengths. figure out from your weakness what makes you want drugs. is it more of a physical or psychological addiction? what are the triggers that make you call for a fix? identify these triggers and take steps to avoid them.

Hey hun sorry to hear you hold slipped back into your weak ways - the best advice I can hand over you is to post photos of your children all around your house and everytime you discern the need to give somebody a lift drugs look at their faces first and enlighten yourself how much they need you surrounded by their life - it is your children who will suffer the most and how just is it on them to have a drug user father or worse still a dead drug user father.
