Answers: Since you're using the IDC, if you're considering whether on not to diagnose, if you have concerns in the region of specific behaviors that don't fit well into any of the other assault categories in the order of or maybe you don't exactly know the specifics (adult client not competent to verbalize complaints) but you're pretty sure it's abuse, use this code (Abuse, Not Otherwise Specified). I use the DSM-IV myself, but next to the nonspecific codes, it's usually used to place anything that doesn't quite fit into any other diagnostic category.
Legal definition usually depends on the state (or country) you live within. Typically, any physical abuse cause illness or persistent marks (bruises or other skin results lasting several hours) or any forcing of sexual act (even if it's your spouse). May also include emotional or choral abuse such as controlling behaviors, humiliation, threatening -- again this would depend on local/state law and regulation.

Maltreatment is abuse or inattention. Can be emotional, physical or sexual.
